Modern GP Access model
The practice has adopted NHS England’s recommended model for patients to request care from their GP practice. This is called the Modern GP Access model. You can get information about this on NHS England’s website using this link.

This new model will be different to the old method of a patient telephoning their GP practice to book an appointment in the following ways.
- You will be asked to give a description of your health concern in an online form.
- We will look at your form within 1 working day and book an appointment with an appropriate clinician in the practice or recommend that you contact a different NHS provider for example community pharmacy. This process is called triage.
- Triage involves two aspects.
- Assessing how urgent your health concern based on the information you have submitted to book an appointment within an appropriate timescale.
- Determining which practice clinician or whether another NHS provider is most appropriate to give advice on your health symptoms or concern.
